Was that topic created before you had vanity urls enabled? That can also happen when you allow people to create topics outside of the group discussion tab, but if you have vanity urls enable then that shouldn't happen.

Yes, agreed. I also recommend that the group name and the vanity url remain separate. Changing a group name should not change a group's vanity url.

I would think that anyone can create groups and the creator can request a name change but a moderator has to apporve the change would be the best way. It would prevent a name change to be in negative way. And yes member should be notified.
